ABSTRACT

To efficiently remove all recurrent lymph nodes (rLNs) and minimize complications, we developed a combination approach that consisted of 68Gallium prostate-specific membrane antigen (PSMA) ligand positron emission tomography (PET)/computed tomography (CT) and integrated indocyanine green (ICG)-guided salvage lymph node dissection (sLND) for rLNs after radical prostatectomy (RP). Nineteen patients were enrolled to receive such treatment. 68Ga-PSMA ligand PET/CT was used to identify rLNs, and 5 mg of ICG was injected into the space between the rectum and bladder before surgery. Fluorescent laparoscopy was used to perform sLND. While extensive LN dissection was performed at level I, another 5 mg of ICG was injected via the intravenous route to intensify the fluorescent signal, and laparoscopy was introduced to intensively target stained LNs along levels I and II, specifically around suspicious LNs, with 68Ga-PSMA ligand PET/CT. Next, both lateral peritonea were exposed longitudinally to facilitate the removal of fluorescently stained LNs at levels III and IV. In total, pathological analysis confirmed that 42 nodes were rLNs. Among 145 positive LNs stained with ICG, 24 suspicious LNs identified with 68Ga-PSMA ligand PET/CT were included. The sensitivity and specificity of 68Ga-PSMA ligand PET/CT for detecting rLNs were 42.9% and 96.6%, respectively. For ICG, the sensitivity was 92.8% and the specificity was 39.1%. At a median follow-up of 15 (interquartile range [IQR]: 6-31) months, 15 patients experienced complete biochemical remission (BR, prostate-specific antigen [PSA] <0.2 ng ml-1), and 4 patients had a decline in the PSA level, but it remained >0.2 ng ml-1. Therefore, 68Ga-PSMA ligand PET/CT integrating ICG-guided sLND provides efficient sLND with few complications for patients with rLNs after RP.

ABSTRACT

Objective To investigate the relationship between vessel cancer embolus with other clinicopathological features and tumor tissue KRAS gene mutation in the patients with colorectal cancer and its significance to colorectal cancer prognosis.Methods The postoperative clinicopathological data in 182 patients with colorectal cancer during 2011-2014 were retrospectively analyzed and the patients were followed up.Results Forty-five cases showed vessel cancer embolus positive of colorectal cancer,which had the rrelationship with the lymph node metastasis,pathological differentiation degree,liver metastasis,nerve infiltration and increase of CEA and CA199 levels,while had no relationship with the age,sex,tumor size,infiltration depth and pathological type.The 3-year survival rate was 51.1% in the vessel cancer embolus positive group,which was significantly lower than 61.3% in the negative group (P0.05).Conclusion Vessel cancer embolus decreases the 3-year survival rate and affects the prognosis in the patients with colorectal cancer.The survival rate in complicating KRAS gene mutation is similar.

ABSTRACT

It has been reported that the high expression of gastrins and their receptors promotes some colorectal neoplasms cells proliferation and inhibit apoptosis.MicroRNAs (miRNAs) are closely related to gene expression regulation in colorectal neoplasms.Researches show that miRNAs have multiple intersections in the regulatory network of colorectal neoplasms with the signal transduction pathway of gatrins which controls the proliferation of colorectal neoplasms.Gastrins perhaps control the proliferation of colorectal neoplasms cells through miRNA signal transduction pathways.These findings have greatly expanded the understanding of the pathoge nesis of colorectal neoplasms and will provide new ideas and methods for the diagnosis and treatment of colorectal neoplasms.

ABSTRACT

Objective To get the basic data of nasal figure of the Han nationality individuals in Xi'an area and provide for junsprudence and the reconstruction of skull. Methods Nasal height, length, depth and breadth of 313cases in Xi'an area, which had different age and sex, were measured by magnetic resonance imaging (MRI). Results Image of MRI could clearly show the figure of nose and the position we selected were correct and accuracy. The specific data were: Nasal length (male:34. 47±4.29 ~52.20±3.47, female:33. 11±3.33~46. 94±3.83); Nasal height(male: 39.22±3.68~59.49±2.30, female: 33.89±3.95~51.75±3.68); Nasal depth(male: 11.89±1.76~16.68±2.48, female: 10.69±1. 81~16.46±2.04);Nasal breadth(male: 33. 09±3. 83~42. 49±2.72,female:32.00±1.94~38. 86±2.61). So the results were credible. Conclusion The nasal figure of individuals in Xi'an area is different as their different age and sex. It promotes that the influence factors of age and sex must be considered in the facial reconstruction and medico legally reconstructing skull.
